Families who have been approved for GAPP (GA Pediatric Program) may get paid for Personal Support services!

How to Get Started

Initial Call and In-Take

This initial step involves gathering information for a preliminary assessment. Next, we schedule an in-home meeting with our nurse, the client, and their family. During this visit, we conduct a thorough needs assessment and develop a personalized care plan.

Physician Approval

We send a Physician Plan of Treatment to your primary care physician for review and approval. Once the physician signs off, we submit all your documents to your insurance for final approval.

Caregiver Match

Once you are approved, we will begin matching potential caregivers to your family using a specialized algorithm designed to ensure the best fit. We ensure a good match by introducing our caregivers to the family before making an assignment. Caregivers maintain medical charting and detailed notes of every visit. To maintain compliance and high-quality care a supervisory nurse conducts visits during care hours.
